Social Security Death Index
This is an index of records of deaths reported to the Social Security Administration. Covers deaths from 1962 to the present. Generally provides dates of birth and death, and possibly the residences at time of death and time of issuance of the Social Security Number. Also provides information on ordering a copy of the "Application for a Social Security Number", which can include birth information, and mother's maiden name.
